Key responsibilities
What you’ll own
- Manage patient onboarding and enrollment processes
- Conduct outbound patient outreach and engagement calls
- Verify eligibility and collect required medical records
- Coordinate communication between patients, providers, and care teams
- Track patient progress throughout the care journey
- Maintain accurate documentation in Salesforce and healthcare systems
- Collaborate with leadership on operational improvements
- Support AI-driven workflow and process enhancement initiatives
Requirements
Who qualifies
- Active LVN license in any U.S. state
- 2+ years of remote healthcare coordination, patient engagement, or case management experience
- Experience working in a healthcare startup or health tech environment
- Comfortable making 50–75 outbound calls per week
- Experience with remote work and virtual patient support
- Proficiency with GSuite, Salesforce, and healthcare technology platforms
- Strong communication and organizational skills
- Ability to work in a fast-paced startup environment
Does not qualify if:No active LVN license
No remote healthcare experience
Pediatric-only healthcare background
No interest in outbound patient engagement
No interest in technology or AI-driven processes
Unable to attend occasional Bay Area office visits
Hospital-only experience with no care coordination background
Preferred qualifications
